From MiH Programme to Reducing Maternal and Newborn Deaths in Kenya Programme

The Reducing Maternal and Newborn Deaths in Kenya programme is delivered through three core work streams:

i) Quality of Maternal and Newborn Care– Maternal and Perinatal Death Surveillance and Response (MPDSR): The key focus is strengthening  notification, reporting and review systems for maternal and perinatal deaths

ii) In-service EmOC&NC Training: Focuses on strengthening the capacity of county/sub-national governments to coordinate, deliver and quality assure in-service competency-based training in EmOC&NC and other ‘skills and drills’-based training to strengthen capacity of the workforce

iii) Pre-service training: Facilitates the integration of competency-based training approaches and content in pre-service training curricula (midwifery and clinical officers) and strengthening of faculty capacity to implement the updated curriculum.

Additionally, the programme provides support to institutionalization of a standard national mentorship approach by providing technical support to the National MoH to design and develop a National Mentorship-and Supportive Supervision programme with an focus on EmOC&NC, and support the Reproductive Health Coordinators at sub-national level to implement it.

Key achievements of Reducing Maternal and Newborn Deaths in Kenya Programme to date

Quality of Maternal and Newborn Care– Maternal and Perinatal Death Surveillance and Response (MPDSR):

At the national level, LSTM has supported the standardisation of National MPDSR training materials and the training of 65 National Level TOTs drawn from all 47 counties in Kenya using the standardised National MPDSR training curriculum/materials. The TOTs have been instrumental in cascading MPDSR training at the county level.

TA provided to the National level for the drafting of the 1st Annual National MPDSR report

TA provided to the National MoH for production of the 2nd  CEMD Report

94 health care workers trained on MPDSR from 3 counties in 12 months, enabling counties to officially constitute and appoint County MPDSR committees and hold quarterly meeting to review data.

Technical assistance provided to county MPDSR committees to meet quarterly, review county maternal death data and take practical actions to prevent maternal deaths and improve quality of care

TA provided to counties to produce bi-annual reports submitted to the National Level MOH MPDSR Secretariat.

Maternal and perinatal death audits have increased the profile of maternal and newborn health in counties and implementation of actions has led to improved quality of health care provided to mothers .

In-service EmOC&NC Training:

Healthcare workers’ (HCW) capacity (knowledge and skills) through training and follow-up mentorship to sustain and utilise the skills acquired in managing obstetric complications has improved significantly: EmONC mentors in target facilities’ maternal and newborn health (MNH) units have the capacity to lead mentorship sessions, imparting knowledge and skills, to the frontline HCWs as mentees

Delivered advanced obstetric training course which targets healthcare providers who provide peri-operative and comprehensive emergency obstetric care with aim of   enhancing their capacity for: Decision-making and leadership of maternity care teams; Pre-operative care; Caesarean section; Assisted vaginal delivery and Prevention and management of obstetric anal sphincter injuries

Increased capacity of target counties  to establish EmOC&NC training systems and facilitate sustainable delivery of EmOC&NC training by completing refresher training for existing master trainers and training of new EmOC&NC master trainers ,training of new Course Directors and Quality Assurance officers to support county level training

Pre-service training:

Mapping of diploma midwifery/clinical medicine training institutions and hubs  in Kenya completed

Support was provided to the National Pre-Service Taskforce to review and finalise the pre-service diploma midwifery and clinical medicine (reproductive health) syllabi and curricula By integrating the competency-based EmONC and MPDSR

Eight (8) pre-service super master trainers and 51 midwifery and clinical medicine (reproductive health)educators and instructors trained as trainers of trainers (ToTs) from across the country on the new updated syllabi and curricula

Thirty-five (35) pre-service medical training colleges  across the Kenya have the capacity to deliver the updated syllabi/curriculum (as at Dec 2020)

Protocol for the evaluation of the implementation of the updated midwifery curriculum in pre-service training Kenya Medical Training Colleges developed.

Mentorship:

Support provided for the development of the National mentorship package, which was successfully completed
